Clinical Manager - Registered Nurse

The FCS Clinical Manager is a highly qualified healthcare provider who, through education and experience, possesses a distinct body of knowledge and skills and
applies the art and science of his/her profession. FCS Clinical Manager is in charge of helping implement Family Caregiver Services from a clinical standpoint and supporting the RN Care Managers in the field. He/she applies nursing knowledge and critical thinking in assessing, implementing, and evaluating client responses to actual or potential health problems. He/she is responsible for (1) Providing direct support to the RN Care Managers when reviewing all POCs for completeness and ensuring compliance; (2) check aide plan of care against orders; (3) review TIFs, discharges, and non-hospital holds for thorough documentation and appropriate Communication Notes; (4) ensure supervisory/delegation visits are completed in the appropriate time frame with correct documentation; (5) coordinating case conference weekly; (6) coordinating other client care activities/services to provide the highest quality of client care; (7) will be able to complete and assist with all Pediatric Assessment Tools (PAT) when required for the program; (8) ensure annual clinical documents such as competencies, PATs and HEPs are completed and obtained in the correct time frame. He/she will be able to assist with all clinical duties, including but not limited to skilled assessments to determine eligibility, start of care, supervision of CNA’s, IHSS Attendants, and/or LPN’s, and recertification, transfer, and discharge visits. He/she will be able to generate Plans of Care, Aide Care Plans and assist with all things pertaining to clinical operations. He/she works under supervision of and receives clinical direction from the Director of Nursing. The FCS Clinical Manager will also help with administrative tasks such as, but are not limited to coordination of services, contacting physician offices, requesting outstanding documentation, and review of completeness of records. At COFK, the FCS Clinical Manager (RN) is expected to be proficient and efficient with the use of an EMR system and multiple technological platforms to provide the highest quality of care to the patients and families we serve.

Education/Licenses/Certifications:
  • Bachelor’s Degree in Nursing and two years experience in home care specifically surrounding CNA Program and IHSS Program.
  • Licensed as an RN in the State of Colorado.
  • BLS CPR certification.
  • Specific specialty experience may be required for certain populations/areas.
Experience/Requirements:
  • Ability to read, analyze, and interpret common scientific and technical journals, professional journals, and legal documents.
  • Ability to read and interpret physicians’ orders and notes from other caregivers as well as respond to them verbally and in writing.
  • Ability to effectively present information to clients and employees and community.
  • Ability to respond effectively to the most sensitive inquiries or complaints.
  • Ability to gauge distances.
  • Ability to define problems, collect data, establish facts, and draw valid conclusions.
  • Ability to deal with several abstract and concrete variables. Ability to improvise in a home setting.

  • Skill to attend to multiple tasks at once which are time sensitive.
  • Basic knowledge of Windows operating system and ability to advance computer skills as required.
  • Ability to establish and maintain effective working relationships with fellow employees, the public, and supervisors.

  • Interviewing/hiring clinical staff alongside DON.
  • Training of Care Manager’s with DON oversight.
  • Observe infection control and standard precautions.
  • Performs other duties as assigned by the Director of Nursing.

Working Conditions:
  • Primarily indoor setting with a moderate noise level.
  • Most work is performed in a climate-controlled environment with minimal exposure to hazardous conditions.
Physical Requirements:
  • Ability to regularly sit, stand, bend, kneel, walk, reach with hands and arms, talk and hear.
  • Ability to frequently use hands to finger handle or feel.
  • Ability to frequently lift and/or move up to 15 pounds
  • Ability to use close vision, distance vision, color vision, peripheral vision, and depth perception.
    Ability to adjust to focus.
Machines, Tools, Equipment and Work Aids Used:
  • Typical office equipment including Microsoft Office Suite and EMR system.
